In connection with specimen collection and diagnostic testing, the Medical Provider's authorized test collectors may capture photographic images of patients for the purposes of identity verification, specimen chain-of-custody documentation, and accurate association of test results. These images are stored securely, treated as part of your health record, and handled in accordance with HIPAA and applicable state law. Images will not be used for any purpose other than those stated herein without your express consent.

C. Information We Collect Automatically

We may also automatically collect information using cookies and other data collection tools. Such collection will only occur with the express consent of the Minor's parent or legal guardian ("Authorized Guardian").

By providing consent, the Authorized Guardian acknowledges and agrees that:

• The Medical Provider and its authorized test collectors may photograph the Minor for the purposes of identity verification, specimen chain-of-custody documentation, and test result association;

• Such photographs will be stored securely and used solely for the purposes stated above;

• Photographs and personal information of Minors will not be shared with any third party except as necessary to facilitate laboratory testing, deliver results, or as required by law;

• The Authorized Guardian has the right to request deletion of the Minor's photographs and personal information, subject to applicable record retention requirements;

• All information collected from or about Minors is treated as Protected Health Information (PHI) under HIPAA and handled accordingly.

No personal information or photographic images will be collected from a Minor without the prior documented consent of the Authorized Guardian. The Medical Provider's test collectors are trained to obtain and verify such consent before proceeding with any data collection involving Minors.

10. Do Not Track

The Site does not support Do Not Track at this time. Do Not Track (DNT) is a privacy preference that you can set in your web browser to indicate that you do not want certain information about your webpage visits collected across websites when you have not interacted with that service on the page.
